Our Veterinarians are USDA accredited, which allows them to clear your pets for travel if they meet the travel requirements. The health certificate form is supplied by the hospital. The form requires the following information from the client:

• Current residential address

• Final destination address 

• Up-to-date vaccination records if vaccination shots were received from a service other Glendale Small Animal Hospital
